OTP and questionnaire

OTP

  1. Phone. "Phone verification", hint "Enter the phone number to receive the code", button "Send code". If the number is passed in metadata the code is sent immediately and the phone screen is skipped.
  2. Code. "Enter the confirmation code", button "Confirm". Resend: "You can get a new code in … s", then "Send again".

Code length, entry and send attempts and the pause between sends are settings in the cabinet.

Questionnaire

A multi-page form configured in the cabinet. The user sees "Page … of …", buttons "Next" and "Submit". Fields can include text, list selection, country search, a camera photo ("Tap to take a photo", "Retake", "Use photo") and an address on a map ("Tap to select an address on the map" or manual entry).

While loading, the interface shows "Loading questionnaire…". If the questionnaire fails to load or submit, the check ends with a failure. Details are listed under Failure reasons.
